Step 5: Make an appointment with Accessibility Services 6 - 8 weeks before term starts

(If you will require either ASL or Braille accommodation, please contact our office 3 to 6 months in advance, whenever possible)

If you are a student with a visual impairment and the steps below are not accessible to you, please let us know by email.

Complete an intake form to start the accommodation process . You can print it off and complete by hand, or download/save it to your computer and type in the required fields. You can return the completed form several ways:

You will also need to provide us with documentation of a disability

Confidentiality: Please note that your documentation/diagnosis is not shared with instructors, and that your online student record will not identify you as a student with a disability.

Once we receive both your completed intake form and your documentation, we will contact you to make a phone or Zoom appointment with an Access Specialist.

* Note: if you are not sure how to obtain documentation, please return the intake form and request a "consult" with an Access Specialist to discuss options.
